Problem Reporting
This form allows ITC Support Staff, ITC Monitors
and ITC Coordinators to report trouble instances in ITCs so that a trouble
ticket can be created to resolve the issue in question. In order for the
ITS Help Desk to adequately address the problem in question, please be
sure to:

Fill out all sections of the form below.

Fill out the Problem Description in as detailed
fashion as possible. Indicate any programs running when an error occurred.
Indicate if the problem is related to one machine or a group of machines.

In the event of a systemic problem (i.e. network outage in your ITC) please
call the ITS Help Desk at 319-384-HELP (4357).
